Our nurses, therapists, and staff will education patients and their family of new or existing disease such as diabetes, high blood pressure, congestive heart failure and how to react.

 

     Home Health Care is a cost-effective option for receiving needed health care services in the comfort of your home. Home Health Care has proven to cut   down on readmission to the hospital and saves billions of dollars each year.  Home Health Care can also assist with chronic disease management. Returning to one's home surrounded by family can promote a quicker recovery and improve the quality of life for the patient, their family or caregiver.

 

Q: What type of Home Health Services can be provided in the home?

 

A: Many medical conditions that previously required hospitalization can safely and effectively be treated in the home, some of the basic services offered by CERTIFIED HOMECARE OF SOUTH FLORIDA are:

Q: Who pays for Home Health?

 

A: There are several different payors for Home Health Services such as HMO's, PPO's, and Worker's Compensation. CERTIFIED HOMECARE OF SOUTH FLORIDA is a MEDICARE CERTIFIED AGENCY whose focus is on being experts in providing care to seniors under the traditional Medicare Plan.

 

Q: Do I need my doctor to order Home Health Services?

 

A: YES. All health care provided in the home is under the direct order and supervision of the patient's physician. With a doctor's order we can send one of our trained nurses to evaluate the patient from head to toe and develop a plan of care in coordination with your physician.

 

Q: My doctor is ordering Home Health Services for me can I request CERTIFIED by name?

 

A: Absolutely, and we’d be honored to care for you or your loved one. Federal law gives patients the freedom to choose their health care provider under Medicare.

 

Q: What if I have a problem at night or on the weekend?

 

A: Home Health Care services are not for emergency or full time care; however, CERTIFIED HOMECARE OF SOUTH FLORIDA has an Administrator and Ra registered Nurse on call 24 hours a day, seven days a week for your convenience: 954-524-8003.

 

Q: How is CERTIFIED different from other home health agencies?

 

A: Providing exceptional care, with excellent patient outcomes is paramount to all we do. We publish our outcomes on a quarterly basis and you can receive this information by calling our local office. All patients receive a patient satisfaction survey, in addition to follow up phone calls to ensure their satisfaction. This is part of our ongoing evaluation process to continually improve our patient satisfaction. New programs, technology, and procedures are developed from the feedback we receive and integrated into our services as needed.
