FOUR REQUIREMENTS FOR CARE

The patient must be covered under Medicare Part A

The skilled care must be intermittent and medically necessary as determined by your physician

The patient must be homebound, certified by a doctor

The patient must have a seen their referring physician 90 days prior to or within 30 days after our start of service
